The link below is our policy on new patients. If you have any questions or would like to schedule an appointment please call the office at 316-687-0776 M-T 8am-5pm Friday-8am-12pm.
Please print and fill out the medical release below for each doctor you have seen (primary care or specialists) in the last 10 years. We require all records to be in office before scheduling your new patient appointment. This will help give you the best care possible.
Return completed form by mail to the address listed on the release form or fax it to the office at 316-688-1512 attention medical records.
If you already have your new patient appointment scheduled. Please click the link below print and complete the forms and arrive 15 minutes before your appointment time. We will need to scan your driver license and insurance card so please have those avaialbe. If you cannot make your appointment please call at least 24 hours in advance to cancel or charges may apply that will not be covered by your insurance. Thank you allowing us to take care of your medical needs.
